Diet for IBS

I had a fissurectomy and sphincterotomy'06 but still bleeding and straining when having BM. 1 yr ago saw doc who said fissure healed - could it re-open. I have had IBS & have trouble with fiber use oat bran why am I still straining, bleeding and in pain?

Dietary Advice for IBS 

In case of chronic fissures that are not healing, doctors recommend more aggressive, intrusive measures such as fissurectomy and sphincterotomy. Fissurectomy involves removing the deformed skin and protrusions that are causing a problem, while sphincterotomy involves cutting the muscles of the internal anal sphincter. Post surgery, patients experience bleeding until the wound has completely healed.




Though these methods are popular there is a high risk of later complications. Bleeding and infection can occur. Continued bleeding even after the wound has healed is due to granulation tissue which can require treatment.


Other complications include occurrence of thrombosed haemorrhoids and even recurrence and persistence of fissures. After the surgery you should take care not to sleep on your back and stick to a light diet. Consume foods that are easy to digest and high on fibre such whole wheat grain toast, crackers, yoghurt, soups and plenty of liquids. Some food items such as beans cause gas and should be avoided till the wound has healed completely to avoid complication. Similarly foods that are difficult to digest such as corn and peanuts should be avoided so that no excess pressure is exerted on the bowels. Also avoid foods such as bananas, apples and pears which have pectin in them which binds food together and causes constipation.

Diet for IBS 

All of your problems sound like they can be taken care of with simple diet and lifestyle changes. The first thing to address is that you are straining while passing stools. This needs to be eased out before you can start with anything else. You can take care of this problem by applying a bit of Vaseline on your finger and inserting it about half an inch into your anus. Go to sleep with this, and your morning bowel movement should be smooth. If this remedy works for you, please make it a part of your daily life. Do it at least thrice a day, especially after a bath and before bedtime.

The next thing to take care of is that you are not passing hard stools. You need to add fibre and fluid to your diet twofold and cut back on carbohydrates by half. Instantly increase your intake of all fresh fruits and vegetables, bran, oatmeal and flaxseed. Avoid white bread, processed food, meat and goods that come out of a bakery. This will gradually soften your stools so that you do not have to suffer so much while passing a bowel movement. You should also try to work towards eliminating toilet paper and use water to wash your genitals with your hands. Pat dry with toilet paper, but do not rub, as this can irritate you further.
